Arbors at New Castle Starts "Just Visiting" Program

Learn how monthly events at Arbors at New Castle Nursing Center have helped close the gap on care transitions by giving patients the opportunity to interview home health agencies and choose the one that's right for them. 
Arbors at New Castle Nursing Center’s Just Visiting program is designed to help patients return home and back into the community after their stay. The administrator and staff of Arbors took it upon themselves to make the program even better by joining forces with Quality Insights of Delaware.
The Just Visiting program gives patients the knowledge they need to make a successful recovery when they return home. As part of the program, Arbors offers an event each month where home health agencies that have passed the company’s set of criteria are invited to share their services with patients and their families. This gives patients in an anticipated 30-day discharge window the opportunity to interview 10 to 15 home health agencies to determine which one is the best fit for their needs.
By working with Quality Insights of Delaware, the nursing center has been able to make connections with other home health agencies and providers to let them know about their program and invite others to participate in the monthly event. This collaboration has helped Arbors close the gap on care transitions.
