Nanticoke Memorial Hospital Gets to the Heart of Readmission Rates

Learn how Nanticoke Memorial Hospital partnered with Quality Insights of Delaware to pinpoint the biggest opportunities for reducing readmission rates and to provide patient education and resources.
 
When Nanticoke Memorial Hospital set out to reduce readmission rates, staff needed a starting point. They worked with Quality Insights of Delaware to determine a focus area.
 
Quality Insights and Nanticoke formed a coalition to bring providers together to start the conversation on improving health outcomes. All of the home care agencies, rehabilitation centers, acute care rehabilitation centers and hospices that service Nanticoke’s area met to identify and discuss existing barriers to smooth care transitions.
 
Additionally, Nanticoke implemented teach back methodology before patients are discharged and follow-up phone calls to patients after discharge to ensure that they make and keep an appointment with their primary care physician.
 
To continue patient education after discharge, Nanticoke formed two patient support groups that meet every other month. The groups were originally slated to meet quarterly but the participants wanted to meet more frequently. Quality Insights provided educational handouts for support group participants.
