SBAR Takes Flight at Regal Heights Healthcare & Rehabilitation Center

Discover how Quality Insights of Delaware helped Regal Heights Healthcare & Rehabilitation Center reduce readmissions by guiding them through the use of evidence-based tools and processes.
 
When Quality Insights of Delaware approached Regal Heights about working together to reduce the facility’s repeated readmissions, the administrator jumped at the chance.
 
Quality Insights’ project coordinator suggested that Regal Heights implement the Stop and Watch Early Warning Tool from INTERACT (Interventions to Reduce Acute Care Transfers) as a first step. Regal Heights introduced this tool by asking staff members to complete a Stop and Watch tool if they noticed change in a patient. The tool prompted nurses to conduct an SBAR (Situation Background Assessment Recommendation), which promotes patient safety because it facilitates the sharing of patient information in a concise and structured format.
 
The Stop and Watch tool helped the Regal Heights team look at residents at risk for hospital admission to see what could be done to prevent them from an unnecessary admission. In the first quarter of 2014, Regal Heights staff completed 20 Stop and Watch tools, which resulted in 20 SBARs. Of those 20 residents, none of them had a hospital admission during this timeframe. An additional 10 residents did not have completed Stop and Watch tools during this timeframe and all 10 of them were admitted to the hospital.
 
Though early in Regal Heights’ initiative, Quality Insights has been so impressed with the facility’s progress that we are creating a three-segment video outlining Regal Heights’ success.
