Improving Care Transitions Summit Scheduled for April 9th

With the goal of improving care transitions, the Louisiana Department of Health and Hospitals, in partnership with the Louisiana Hospital Association and eQHealth, invites you to attend a statewide care transitions summit on April 9, 2015 in Metairie, Louisiana. The event will focus promising practices for creating more-effective transitions in care for Medicaid recipients.
The summit will be held at the Sheraton Metairie – New Orleans Hotel on Thursday, April 9, 2015. This all-day event will feature a keynote address by Benjamin F. Miller, Psy.D. and Assistant Professor at the Department of Family Medicine of the University of Colorado Denver School of Medicine, who is a national expert in the field. Local leaders will also share their experiences with cross-continuum coordination strategies that have led to a reduction in preventable hospital readmissions.

Working sessions will provide opportunities for participants to:
  • Discover opportunities and effective strategies for the reduction of hospital readmissions
  • Develop robust partnerships with organizations across the continuum of care
  • Design approaches to address key drivers of Medicaid readmissions, such as behavioral health, substance abuse and unmet non-medical needs for patients with chronic conditions
This summit will feature participation from across the landscape of care. Participants will come from several key sectors, including care management, discharge planning, social work, nursing and emergency department leadership, home health agencies, nursing homes and community organizations.
