In their own words: Louisiana Homecare of Slidell, LLC, Takes Steps to Reduce Rehospitalizations

At Louisiana Homecare of Slidell, our objective is to reduce the incidence of patients returning to the hospital shortly after being admitted to home health. We utilize our clinicians, team leaders, staff RN’s, LPN’s and administrative team to accomplish this. 
We began this project in the Fall of 2010, and it is still ongoing. We assess and front load visits on every person at risk of returning to the hospital. We made additional phone follow ups on patients who were at risk.

We encountered many barriers along the way. They included patient non-compliance, or not understand quickly enough what changes they need to make in their lifestyle. Also, we noticed patient stress levels were affecting their ability to learn.

Overall, we improved many areas, such as decreased returned hospitalizations and better overall outcomes.  We are diligently continuing efforts through persistence.
