New Jersey Care Coordination Community Coalitions

As of November 2017, Quality Insights is facilitating seven active regional coalitions throughout New Jersey to address issues related to care transitions, care coordination and patient safety.
 
The Vision
To build a strong and sustainable community-based health care network that will provide area residents with consistent, patient-centered, quality care targeted to their needs

The Overall Goals

To reduce hospital admission and 30-day readmission rates, increase community tenure, and reduce adverse drug events (ADEs) throughout NJ

How We Do It

Each regional coalition provides a platform and opportunity for Medicare providers and key stakeholders to convene together to share best practices, network across facilities and organizations, and discuss successes and challenges to address community needs.  
 
In each community, coalition members are collaborating to improve the coordination of care that patients receive. Coalition members have identified factors and causes that contribute to less-than-optimal health outcomes, examined transitions of care, developed or enhanced established care transitions interventions and other proven tools and resources and are continuously implementing interventions that improve long-term health outcomes.

Structure and Community Partners

This community uses a unique method to meeting, utilizing virtual meeting structure, allowing for more flexible participation in Coalition meetings.  There is a diverse representation from 91 healthcare and social organizations represented in this Coalition, with key agencies participating, including:
