As one of fourteen regional Quality Innovation Network-Quality Improvement Organizations, we spread best practices for better care while accommodating local settings and cultural factors. No matter what health condition we focus on, our overarching goals are always the same: better care, smarter spending, healthier people.
We offer quality improvement expertise and technical assistance to healthcare providers and partners located in the network of states we serve. Our data-driven quality initiatives engage patients, families and clinicians to improve patient safety, reduce harm and improve clinical care.
Just a few examples of our recent collaborative successes include:

  • Leading community coalitions that helped hospitals reduce avoidable readmissions by nearly 9,000, improving care while saving an estimated $104 million
  • Graduating more than 5,000 Medicare patients from comprehensive Diabetes Self-Management Education (DSME) courses
  • Engaging more than 1,200 nursing homes on quality improvement measures, which resulted in 3,517 fewer residents with Self-Reported Moderate/Severe Pain.
  • Increasing the number of patients taking advantage of the Medicare Annual Wellness Visit (AWV) benefit by more than 160,000 in a two-year period
  • Launching an opioid initiative with physicians’ offices that contributed to nearly 40,000 fewer people with Medicare taking opioids in a one-year period.
