CMS, through its Center for Medicare & Medicaid Innovation (CMMI), launched TCPI. The initiative seeks to assist approximately 140,000 U.S. clinicians (primary and specialty care) in ambulatory and non-acute care settings in transforming their practices to deliver better health outcomes at lower costs. The model will assist clinicians in improving the way they deliver care by providing technical assistance support for integrating quality and process improvement, and by building on and spreading existing change methodologies, practice transformation tools, published literature, and technical assistance programs.

Although a portion of physicians across the United States have already moved to coordinated and value driven care through participation in programs such as ACOs, Medicare Shared Savings Program, Patient Centered Medical Homes, and Advanced Practice Payment Models, a far greater number have yet to transform their practices to move from a volume-based reimbursement model to value-based care. TCPI provides an opportunity for this larger-scale practice transformation.

As the U.S. healthcare system moves from a volume driven, primarily fee-for-service model to multiple integrated care models, Quality Insights is helping clinicians achieve large-scale health transformation. We provide technical assistance to clinicians to assist them in changing the way they deliver care by integrating quality and process improvement to build on and spread existing change methodologies, practice transformation tools, published literature, key learnings and technical assistance programs.

Specifically, Quality Insights partners with Practice Transformation Networks across the country to provide participating practices with:

  • A baseline assessment to help practices get started on their journey to improved quality
  • Follow-up assessments with a limited number of practices to evaluate progress
  • Learning opportunities to assist practices with quality improvement efforts
